A Histological Study on the Endomentrial Response to Copper-and Hormone-Releasing Intrauterine Devices

Abstract
A histopathologic study on the endometrial response to copper-and hormone- releasing intrauterine devices was performed in order to observe the nature and the incidences of abnormal endometrial findings. Devices used in the present study were Cu-7(200) and Alza-T which releases 50 micrograms of natural progesterone per day. Endometrial biopsies were obtained from 51 Cu-7 users and 46 Alza-T users on their 12-month follow-up visits and the following results were obtained; 1) In 25.6 per cent of the Cu-7 users and 26.5 per cent of the Alza-T users, the histological dating did not correspond to their menstrual cycle day, showing endometrial changes, either advanced or retarded 4 days or more from the menstrual cycle day calculated from their last menstrual period. 2) The hyperplastic change was observed in 15.4 percent of the Cu-7 users, whereas the same finding was observed in only 2.9 percent of the alza-T users. 3) The pseudodecidual reaction was not observed in the Cu-7 users, while 23.6percent of the Alza-T users showed the pseudodecidual reaction. 4) The inflammatory response was frequently observed in the both groups, and 59.0% of the Cu-7 users and 73.5% of he Alza-T users exhibited significant degree of small round cell infiltration. 5) Definite decrease of the PAS-positive substance in the endometrial glands was observed in 23.5% of the Alza-T users, whereas its decrease was not clearly manifested in the Cu-7 users.
